Student-Athlete Clearance
Before participating in a sport, students must be cleared by the Athletic Director.
Students must print out, complete, and turn in the following documents to the head coach:
- Complete Online Clearance at athleticclearance.com
- Must upload a copy of health insurance card to move on
- LAUSD Student Emergency Card
- Physical Exam Form
- Self-Administration of Medication form and Asthma Action Plan must be attached to physical if student has asthma
Process for clearing attendance for students missing classes due to home/away games:
- Email faculty staff dismissal time for each campus, along with roster
- Students must be wearing jersey or gameday attire

Students must change and be dressed in uniform prior to leaving on bus (can change in Flagship locker rooms- not restroom)
Attendance Taking Procedures:

At or immediately following the event, the activity director/coach/sponsoring teacher must create a note for the Attendance Office at Flagship/Front Desk A Building at Olguin.

That note must include:

What the activity was (athletic event, field trip, blood drive, etc - this information is needed for proper coding in misis)

A list of students who participated in the school activity/field trip/athletic event (alphabetical order kindly appreciated by our office staff)Please Note: The Attendance Office cannot use the "Please Excuse the following students" email often sent in advance of the activity (The reason is that it only specifies which students are expected to participate. In order to excuse an absence, the Attendance Office must have verification of which students actually participated.)
- The date and periods missed as a result of the event

The name and signature of the activity director/coach/sponsoring teacher

That note should be turned into Ms. Almeida in the Flagship Attendance Office or the front desk in the A Building at Olguin (one list can be submitted to the office at the teacher's/staff member's main location, even if students from both campuses participated)
